Step one
Ask our office about joining the practice. Membership prices and policies here are illustrative and can be customized for your practice.
Step two
Your request is approved once your intake forms are in and reviewed. Then your time is confirmed.
Step three
Sixty minutes with Dr. Rivera in the office, to go over your health, your history and your questions.
Step four
Book online yourself: an annual visit of an hour, or a follow-up or sick visit of thirty minutes. Choose the office or a video visit on Zoom. We send an email reminder a week before, and texts a day before and two hours before.

There is a one-time $29 registration fee to sign up. After that, the monthly price is set by age: $39 for ages 13 to 17, $59 for ages 18 to 34, and $79 for ages 35 and up.
Membership is billed monthly, and payments are made online through Stripe.
Yes. $149 a month covers two adults in the same household when both are 35 and up.
Membership care is not billed to insurance, and there is no copay at the desk for membership visits. Insurance is still wise to keep for hospital care, specialists, emergencies and imaging.
New patients send a request for a 60-minute New Patient visit in the office. We approve it once your intake forms are received and reviewed.
Current patients book instantly online, at least 4 hours ahead and up to 90 days out. Reminders come by email 7 days before, and by text 1 day and 2 hours before, with a request to confirm.
Monday to Thursday, 7:00 AM to 5:00 PM, closed 12 to 1 for lunch. We are closed Friday, Saturday and Sunday.
We are also closed on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas.
Yes. Annual visits, follow-ups and acute or sick visits can be done by video on Zoom, which helps when you’re too sick to come in or out of town. Your first visit as a new patient is in the office.
There is a 10-minute grace period. After that, we’ll need to rebook your visit. Please give us 24 hours’ notice to cancel.
No. SoftWave therapy, a non-invasive acoustic wave treatment, is offered separately and priced separately from membership. Ask Dr. Rivera or call the office to learn more.
